We value referrals from Medical Practitioners and Allied Health Professionals and pride ourselves on providing a valuable support service to enhance the care you provide patients living with:
- Diabetes and associated renal and hepatic conditions
- Cardiovascular disease and associated renal and hepatic conditions
- Gastrointestinal diseases and conditions
We offer consultations in our Hawthorn East, South Yarra and Mount Waverley rooms, or via telehealth.
Following a thorough medical assessment including clinical and biochemical data, our dietitians support patients through evidence based dietary advice and ongoing practical recommendations.
Our interventions focus on modifiable dietary and lifestyle risk factors. Realistic and achievable recommendations are tailored to complement prescribed medication, the patient’s living circumstances and their readiness for change.
Consultation fee and rebate information
Melbourne Dietetics Clinic is a private practice and not a bulk billing service.
Our fees are as follows:
Initial Consultation (allow 60 minutes) $270
Review Consultation (allow 30 minutes) $140
We offer Medicare and Private Health Cover rebates for eligible patients and can process Medicare claims on the patient’s behalf at the time of consultation.
If your patient has a GP Chronic Condition Management Plan in place (previously referred to as an Extended Patient Care (EPC) Plan), they are eligible for a Medicare rebate on our service. ($60.80 per consultation) We would appreciate you providing a copy at the time of referral, to ensure continuity and patient centred care.
Some Health Insurance Policies cover part of our fee, depending on extras included in individual policies. Rebates vary among Insurers so check the patient will need to check with their insurance provider regarding eligibility for rebates.
